First Families is a free founding membership program at The Village Vet+ Annex. It is our way of recognizing the first 200 local households that choose us for their pets' care. These terms explain who can join, how the 15% discount works, and how membership is kept.
The program is offered by M. Astin Veterinary Medicine Professional Corporation, doing business as The Village Vet+ Annex. In these terms, we refer to the clinic as "we", "our", the "clinic" and "TVV+ Annex."
Who can join
The First Families membership is limited to the first 200 qualifying households. Membership is free.
One membership is available per household. It is linked to one primary client account and the household's current address. A household includes the primary member, their spouse or partner, dependent children, and all pets that normally live at that address.
How a household qualifies
A household qualifies when all of the following have happened:
- A pet has completed its first appointment at TVV+ Annex.
- The appointment has been paid for in full.
- TVV+ Annex has received the pet's previous medical records, if those records exist.
- TVV+ Annex has been designated as the pet's primary veterinarian.
- A First Families spot is still available.
We will use the dates and times in our clinic records to decide the order in which households qualify. Sending an appointment request or booking an appointment does not, by itself, secure a spot.
If a pet has previous records but TVV+ Annex has not received them by the first appointment, we may hold the household's place for up to 30 days after the paid appointment. If the records arrive during that time, the household may qualify based on the date of the appointment. If the records arrive after 30 days, the household may still qualify if a spot remains available.
Previous records are not required for a puppy, kitten, or recently adopted rescue pet when no previous records are available. TVV+ Annex may ask for reasonable information about the pet's age, adoption, or known medical history.
TVV+ Annex will confirm when a household has received First Families status.
The First Families benefit
Active First Families members receive 15% off the eligible subtotal of all services on their TVV+ Annex invoice. The discount is applied before tax. It covers professional veterinary services provided at TVV+ Annex, unless an exclusion below applies.
Professional veterinary services specifically includes: examinations, surgical procedures, laboratory diagnostics and radiology.
If TVV+ Annex has received the required records by checkout, the discount can apply to the pet's qualifying first appointment. If the records arrive later and the household qualifies, TVV+ Annex will add an account credit equal to the discount that would have applied to the eligible items on that invoice. The credit has no cash value and can be used only toward a future TVV+ Annex invoice.
What is not discounted
The First Families discount does not apply to:
- Taxes.
- Gift cards.
- Wellness plan membership fees.
- Services or products already discounted through a wellness program or other promotion.
- Outside specialist or third-party charges that TVV+ Annex passes through.
- An insurance deductible, co-pay, reimbursement, coverage amount, or any other amount set by an insurer.
- Any item that is already receiving another promotion or discount, in which case, the First Family member will benefit from the more favorable discount.
- Products sold by TVV+ Annex, including food, retail and pharmacy.
Other promotions and discounts
Discounts cannot be stacked. If an item qualifies for more than one offer, only one discount will apply. TVV+ Annex will apply the offer that gives the greater savings where practical. Eligible full-price items on the same invoice may still receive the First Families discount.
The First Families benefit cannot be exchanged for cash. It cannot be used to create a cash refund or a negative invoice balance.
Pets covered by the membership
There is no set limit on the number of pets covered. The membership covers every current and future pet that normally lives at the primary member's home. TVV+ Annex may ask for each pet's relevant previous medical records when those records exist.
The discount cannot be used for pets that belong to friends, relatives, clients, or other people outside the household. A pet stops being covered when it no longer normally lives in the household.
Moving to a new home
The membership follows the primary member if they move. The primary member must give TVV+ Annex their new address. After the move, only pets normally living at the new primary residence are covered.
Medical records and primary care
Clients may ask for copies of their pets' medical records. TVV+ Annex will share records as required for safe care and under the rules that apply to veterinarians practicing veterinary medicine in the Province of Ontario.
Sharing records for an emergency visit, referral, specialist, second opinion, insurance claim, travel, or a similar reason will not, by itself, affect First Families status.
If records are sent to another general veterinary practice, TVV+ Annex may review the membership. TVV+ Annex may remove First Families status if we reasonably believe that TVV+ Annex is no longer the household's primary veterinary clinic. Removal is not automatic. We may ask the primary member to confirm where the household receives its regular veterinary care, and we may maintain or restore membership when we believe it is fair to do so.
How long membership lasts
The benefit continues for the primary member's lifetime, and provided that M. Astin Veterinary Medicine Professional Corporation owns and operates TVV+ Annex. If TVV+ Annex moves to another location and continues to be owned and operated by the same company, the membership will continue at the new location.
The program and its benefits will end if TVV+ Annex permanently closes or M. Astin Veterinary Medicine Professional Corporation no longer owns or operates the clinic, unless the company or a new operator confirms that the program will continue.
Transferring membership
Membership cannot be sold, given away, or transferred to another household. If the primary member dies, a spouse or partner who was already listed on the account and lived in the household may ask to become the new primary member. TVV+ Annex may ask for information needed to update the account.
When TVV+ Annex may remove membership
TVV+ Annex may suspend or remove First Families status for a serious or repeated breach of these terms, including:
- Fraud or false information.
- Sharing the discount outside the household.
- Unpaid account balances.
- Serious or repeated violations of TVV+ Annex's conduct or safety policies.
- Moving the household's primary veterinary care to another general veterinary practice.
Where reasonable, TVV+ Annex will explain the concern and provide the primary member an opportunity to correct it before membership is removed. TVV+ Annex may act right away where there is fraud, a safety concern or another serious issue.
Changes to these terms
TVV+ Annex may make reasonable administrative changes, correct errors, or clarify these terms. We will not reduce the core 15% benefit for an active First Families member while the program continues. If a change could affect how a member uses the program, we will provide reasonable notice.
Privacy
TVV+ Annex will use and protect personal information according to its privacy practices and applicable law. Information may be used to manage the client account, confirm eligibility, provide veterinary care, and administer the First Families program.
